Hypertension Virtual Care Program Overview

Program Objective

Codex Health has partnered with your healthcare provider to provide you support  in managing hypertension (high blood pressure).

  • The Codex Virtual Hypertension Program aims to help people diagnosed with hypertension (high blood pressure) to achieve and maintain a target blood pressure (BP) of less than 130/80 mmHg. This target is based upon recommendations from the American College of Cardiology (ACC) and the American Heart Association (AHA).
  • Codex Clinical Educators will support you with BP monitoring, medication management, lifestyle changes, and individualized education throughout the program. A care plan will be developed for you, and presented as a daily task list in the Codex developed, AllieTM app. Consider Allie your ally helping you stay on top of your health.
  • The AllieTM mobile app enables your healthcare provider and a Codex Clinical Educator to see and review your BP data.  The BP data is accessed retrospectively (not in real time) and used by your healthcare team to make decisions, with you, about  your care.

Blood Pressure Monitoring

  • To establish a baseline, or average, reading during the first week of enrollment, you will be asked to check your BP twice in the morning, one minute apart and twice in the evening, one minute apart (for a total of up to 12 measurements).
  • After establishing a baseline, you will be asked to measure BP once a day until your BP is less than 130/80 mm/Hg or less.
  • It is important to take your readings daily and at the same time each day.
  • BP fluctuates over a 24-hour cycle, and changes when you are excited, nervous, active, and according to your body’s natural rhythms. Individual BP readings may vary widely, and a single high reading is not necessarily cause for concern. Having a consistent record of readings over time provides the best overall picture of your blood pressure. We look at patterns and averages to help guide clinical decision making.

Coaching Sessions

  • An essential part of the program is scheduling a monthly 15 minute coaching session to speak with the Codex Clinical Educator. Once monthly is the minimum, but you may schedule a session anytime you like. Coaching sessions are unlimited. You may also message the Codex Clinical Educator directly through the AllieTM app at any time.
  • During the coaching session, the Clinical Educator will answer your questions, review your BP data, discuss lifestyle interventions, monitor medication adherence and provide individualized education and support.
